PTA SOAP 노트 실전 문장 – Plan(P) 이렇게 쓰면 안전합니다
Plan(P)는 단순히 “다음에 뭘 할지”를 쓰는 공간이 아닙니다.
치료의 연속성, 숙련된 서비스의 필요성, 향후 방향성을 명확히 보여주는 영역입니다.
✔ Plan(P) 작성 핵심 원칙
- 미래 시제(Future tense) 또는 예정 표현 사용 ⭕
- PT 처방 범위(Scope) 벗어나지 않기 ⭕
- Frequency, Progression, Education 포함 ⭕
- “지켜볼 예정” 같은 모호한 표현 ❌
1️⃣ Frequency & Duration (치료 빈도)

Patient will continue physical therapy 2-3 times per week as per plan of care.
→ 처방된 치료 계획에 따라 주 2~3회 물리치료를 지속할 예정임. -
Treatment frequency will remain unchanged at this time.
→ 현재 시점에서 치료 빈도는 변경 없이 유지할 예정임. -
Therapy sessions will continue to address functional limitations.
→ 기능적 제한을 해결하기 위해 치료 세션을 지속할 예정임.
2️⃣ Progression (운동 및 활동 진행 계획)

Therapeutic exercises will be progressed as tolerated.
→ 환자의 내성에 따라 치료 운동을 점진적으로 진행할 예정임. -
Balance activities will be progressed to reduce fall risk.
→ 낙상 위험 감소를 위해 균형 활동을 단계적으로 강화할 예정임. -
Gait training will be continued with increased challenge as appropriate.
→ 필요에 따라 난이도를 높여 보행 훈련을 지속할 예정임. -
Resistance will be increased gradually based on patient response.
→ 환자 반응에 따라 저항 강도를 점진적으로 증가시킬 예정임.
3️⃣ Patient Education (환자 교육)

Patient will continue home exercise program as instructed.
→ 안내된 가정 운동 프로그램을 지속 수행할 예정임. -
Education on proper body mechanics will be reinforced.
→ 올바른 신체 사용 방법에 대한 교육을 반복 강화할 예정임. -
Fall prevention strategies will continue to be reviewed.
→ 낙상 예방 전략에 대한 교육을 지속적으로 제공할 예정임. -
Patient was instructed to monitor symptoms and report changes.
→ 증상 변화를 관찰하고 보고하도록 교육함.
4️⃣ Safety & Monitoring (안전 관리)

Patient will be monitored closely for signs of fatigue or pain.
→ 피로 및 통증 징후를 면밀히 관찰할 예정임. -
Rest breaks will be provided as needed for safety.
→ 안전을 위해 필요 시 휴식을 제공할 예정임. -
Treatment intensity will be adjusted based on tolerance.
→ 환자 내성에 따라 치료 강도를 조절할 예정임.
5️⃣ 협업 및 다음 단계

Communication with supervising PT will continue as needed.
→ 필요 시 담당 PT와 지속적으로 소통할 예정임. -
Progress toward goals will be reassessed in upcoming sessions.
→ 향후 세션에서 목표 달성 정도를 재평가할 예정임. -
Plan of care will be followed as established by the PT.
→ PT가 수립한 치료 계획을 준수하여 진행할 예정임.
